The Community Health Assessment Survey is now available online in English and Spanish. The purpose of the survey is for Durham County residents to select what they think are the top 5 health priorities for 2018-2021 and share how they want the priorities addressed. We’ll use the results of the survey to help select Partnership committees for the next three years and develop action plans.

Feel free to take the survey if you live in Durham County. More importantly, please share the link with community  members, your networks, patients and clients you work with. We want to spread the survey as widely as possible to make sure everyone’s voice is heard. The survey will be available through February 16. You can share a variety of ways:
